Golden Rule Hospice: Transitional Care The staff at Golden Rule Hospice understands the occasional need for additional care and support. Transitional care is just that and is a unique benefit to our patients. This level of care is usually initiated whenever a hospice patient is declining or doesn’t qualify for continuous care. Keep in mind that continuous care along with routine home care, respite care, and in-patient care are covered by the Medicare hospice benefit. The primary examples of “decline” in a patient that warrant transitional care include: • comatose or unarousable state • decreased urine output • decreased viral signs such as blood pressure or pulse and irregular respirations • increased temperature When a patient exhibits these symptoms of decline, the family is contacted and asked if they would like this level of care.
