Golden Rule Hospice: Transitional Care
The staff at Golden Rule Hospice understands the occasional need for additional care and support. Transitional care is just that and is a unique benefit to our patients. This level of care is usually initiated whenever a hospice patient is declining or doesn’t qualify for continuous care. Keep in mind that continuous care along with routine home care, respite care, and in-patient care are covered by the Medicare hospice benefit.
The primary examples of “decline” in a patient that warrant transitional care include:
• comatose or unarousable state
• decreased urine output
• decreased viral signs such as blood pressure or pulse and irregular respirations
• increased temperature
When a patient exhibits these symptoms of decline, the family is contacted and asked if they would like this level of care. To learn more about transitional care, contact Golden Rule Hospice today.
